HEALTH & SPAY/NEUTER CONTRACT FOR PET/COMPANION PUPPY/DOG

This agreement is made and entered into this _______ day of _______________ 20____

by and between Rocky & Pamela Mixon owners of Southern Huskies (Breeder), and

for the purpose of setting forth the terms and conditions of the purchase by the Buyer of a

pet/companion purebred Siberian Husky from the litter whelped on _________________

For $______________, the Breeder agrees to sell and the Buyer agrees to purchase a

Siberian Husky pet/companion puppy from the litter described above subject to the following terms.

• Breeder warrants that the above described puppy/dog is in good health and has received
  

age appropriate vaccinations and de-worming medication(s) at the time Buyer assumes ownership.

• Buyer agrees to have the puppy examined by a licensed Veterinarian within 72 hours
  

(excluding weekends and holidays) of receipt of puppy/dog.

         If, as a result of the examination, the Veterinarian determines that the puppy/dog

         is not in good health then the Buyer shall return the puppy/dog to the Breeder for a         

         refund of the purchase price (excluding shipping) or a replacement puppy, providing

         the puppy is accompanied by a written certificate of ill health from a licensed Veterinarian. 

         If a replacement puppy is not available at the time of return of said puppy, full refund of

         the original purchase price (excluding shipping) will be given. All paperwork, health

        records, and Registration papers that was given to the buyer when the puppy

         was sold must be returned with the puppy at that time. The breeder will NOT be

         responsible for any debts or expenses incurred by the Buyer/owner(s) for any reason.

         Buyer shall be obligated to return the puppy/dog to the Breeder at Buyer expense.

         All action taken to return the puppy/dog shall be done with the best interest of the

         puppy/dog in mind.

• If at anytime during the first 3 years of the puppy/dog’s life, if he/she is examined by a
  

Certified Veterinarian, and is diagnosed with any genetic/hereditary or congenital disease

prong to the Siberian Husky the Breeder requires the Buyer and their Veterinarian to forward

all findings to the Breeders Veterinarian within 2 weeks of diagnosis for review and possibility

a second Veterinarian for a second review. If the findings are confirmed genetic/hereditary or

congenital and not due to age, neglect, or abuse the breeder will either:

                                       1.) refund the Buyer the full purchase price.

                                       2.) will give the Buyer another puppy of equal value if available.

                                    We will not require the owner’s at any time to return the dog .

• The Breeder has determined that the puppy/dog is of “pet quality” and is sold as a pet and
  

family companion with Limited Registration and is not to be used for breeding purposes. Therefore,

the Buyer agrees to have the  puppy/dog spayed/neutered by the age of 7 months.

                                 This does not mean that this puppy is an inferior dog.

                                 It is capable of becoming a lovable, well-trained, and

                                 perfectly happy dog. Breeding is not to be done casually

                                nor is it a money-making venture. It is very time consuming,

                               expensive, and demanding.

• The Buyer agrees that if for whatever reason they cannot keep or take proper care of the
  

puppy/dog, the Buyer must contact the Breeder and the Breeder will either: 

                                       1.) take the puppy/dog back with no refund to the buyer

                                       2.) assist the Buyer in finding a suitable home for the puppy/dog.

• Buyer agrees that the puppy/dog will reside at the home of the Buyer, have access to a
  

fenced yard  (or plenty of daily exercise), not to be chained or tethered, and will be provided with

proper housing, a good quality diet, annual checkups by a licensed veterinarian, required

vaccinations, including rabies, and monthly heartworm preventatives.

• It is understood and agreed that the Buyer, by signing this contract has read and understands
  

the provisions of this contract and will abide by it fully. Failure to comply with all aspects of this

contract renders all guarantees null and void.
